HealthPro Heritage in Guilford, CT is seeking a Home Health Physical Therapist to join our patient-focused team. This role supports care across Guilford and surrounding areas, with flexible PRN schedules and a strong collaboration with Masonicare home health.

Responsibilities include patient assessment, treatment planning, documentation, and advocating for clients’ needs while delivering high-quality, compassionate care in diverse settings.

Qualifications

• Degree in Occupational Therapy from an accredited institution.
• Valid state licensure as an Occupational Therapist, or license eligible.
• Strong clinical assessment and diagnostic skills; ability to develop and implement effective treatment plans.

• Patient Assessment: Evaluate clients' physical, emotional, and cognitive abilities using observations and standardized tools.
• Treatment and Education: Create personalized treatment plans to meet clients' goals and educate on activities and adaptive equipment.
• Documentation: Maintain accurate records of evaluations, progress, and plans.
• Advocacy and Support: Advocate for clients' needs and connect them with community resources.

• Point base rate: $65-$75 per point

Qualifications

• Education: Degree in Occupational Therapy from an accredited institution.

• Skills: Strong clinical assessment and diagnostic skills. Proficient in developing and implementing effective treatment plans. Excellent communication and interpersonal skills with a patient-centered approach.
• Physical Requirements: Ability to lift and assist patients as needed. Must be able to stand for extended periods and perform physical tasks related to therapy.
