Job summary

HealthPro Heritage is seeking a Home Health Occupational Therapist to serve Danielson, CT and surrounding areas. This position offers part-time and PRN options in a home health setting for adult patients, with scheduling built around the clinician's availability.

Your responsibilities include patient assessments, creating and guiding therapy plans, documenting progress, and advocating for client needs while collaborating with the Masonicare home health partnership.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• State licensure as an Occupational Therapist or eligibility to obtain licensure.
• Strong clinical assessment and diagnostic abilities.
• Proven ability to develop and implement effective treatment plans.
• Excellent communication and interpersonal skills with a patient-centered approach.

Responsibilities

• Patient assessment of physical, emotional, and cognitive abilities using observations and tools.
• Develop and implement personalized treatment plans to meet goals and improve function.
• Maintain accurate documentation of evaluations, progress, and plans per standards.

Compensation:

• Estimated total annual pay range: $54,080 - $62,400
• Point Base Rate range: $65-$75 per point

Qualifications

• Education: Degree in Occupational Therapy from an accredited institution.
• Licensure: Valid state licensure as an Occupational Therapist, or license eligible
• Skills: Strong clinical assessment and diagnostic skills. Proficient in developing and implementing effective treatment plans. Excellent communication and interpersonal skills with a patient-centered approach.
• Physical Requirements: Ability to lift and assist patients as needed. Must be able to stand for extended periods and perform physical tasks related to therapy.
