Job summary

ABL Health Care is seeking a compassionate, organized Occupational Therapist (OT) for our Home Health team in the Brentwood, Antioch, Oakley, Bethel Island region. This full-time role involves evaluating patients, developing and implementing care plans, and coordinating with the Clinical Director and staff to deliver high-quality in-home care.

The OT will travel within a 15–25 mile radius, contribute to patient education, and help maintain optimal functional status in patients’ homes.

Qualifications

• Current OT license to practice in California is required.
• Experience in Home Health or Hospice care is preferred (1 year minimum).
• Valid California driver’s license is required.

Responsibilities

• Provide skilled intermittent care in the patient’s home and educate patients and families to promote maximal functional status.
• Regularly evaluate and re-evaluate patient status and adjust care as needed.
• Administer treatments, monitor conditions, and report changes to appropriate clinicians.
• Initiate and revise the patient’s plan of care (POC) and address issues identified in the POC.
• Maintain accurate clinical records and documentation per policy using a company device.
