Job summary

Elara Caring is seeking an Occupational Therapist to deliver home health services across their service area in Michigan. The role requires evaluating patients, developing and implementing plans of care, and coordinating with a care team to ensure quality, safe discharge planning, and ongoing home-based therapy.

The position entails 50% travel, a strong clinical background in OT, and the ability to guide and supervise staff while addressing patient needs and safety at home.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted OT license valid for the state of work.
• Minimum one (1) year of experience in a clinical care setting.
• Able and willing to travel within branch/office coverage area.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel 50%
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

Responsibilities

• Evaluates the occupational therapy patient to determine rehabilitation needs and develops a plan of care.
• Implements Plan of Care and ensures quality care and discharge planning; conducts initial and re-evaluation visits and follow-ups per guidelines.
• Communicates significant findings to supervisor, physician, and care team.
• Initiates patient safety plans using family and community resources.
• Verifies Plan of Care prior to visits and follows physician orders and standards.
• Writes orders to cover additional visits and changes to the Plan of Care.
• Incorporates patient goals into therapy care as documented.
• Performs OT services per standards and physician certification.
• Instructs patients/families in activities of daily living with adaptive equipment and therapy programs.
• Assists in securing necessary equipment, including splints if needed.
• Instructs patients in home therapy programs and provides modifications as needed.
• Counseling and instruction to patient and family; supervises COTA, HHA, and staff.
