Job summary

Angels Care Home Health is seeking an Occupational Therapist to perform clinical assessments, develop and maintain a Plan of Care, and adapt treatment plans as patient needs evolve. This full-time position supports a Monday through Friday schedule with vehicle allowance and on-call pay, plus comprehensive health, dental, and vision benefits.

The role requires an OT license, two years of experience, CPR and Hepatitis screening, and knowledge of durable medical equipment.

Qualifications

• Two years of experience as an Occupational Therapist.
• Current state license as an Occupational Therapist.
• CPR certification and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Perform clinical assessment; identify patient needs and appropriateness of patient.
• Document, prepare, and complete a Plan of Care, progress notes, and other clinical record documentation on an ongoing basis.
• Revise the POC based on evaluation and ongoing assessment.
• Apply infection control and universal precautions in coordinating/performing patient care.
• Accept assignments consistent with education and competence to care for patients.
• Use effective interpersonal relations and communication skills to aid team outcomes.
• Assess physical, social, and cultural components of the patient’s occupational environment.
