Job summary

Innovive Health is seeking a compassionate Occupational Therapist (OT) to join our Home Health team. You will deliver skilled OT services at patients' homes, evaluate needs, and create individualized plans to improve independence and safety.

You will collaborate with physicians, nurses, and the care team to set goals, determine visit frequency, and provide evidence-based interventions. A valid MA OT license and CPR are required, with travel across the service area anticipated.

Qualifications

• Graduate degree in Occupational Therapy from an accredited program.
• Current and unrestricted Massachusetts Occupational Therapist license.
• Current CPR certification.
• Minimum of 1 year of experience in home health, skilled nursing, acute care, or inpatient rehabilitation.
• Valid driver's license, reliable transportation, and active automobile insurance.
• Ability to travel throughout assigned service area to conduct patient visits.

Responsibilities

• Conduct home-based occupational therapy evaluations, follow-up visits, 30-day reassessments, recertifications, and discharge visits.
• Assess patients' functional abilities, rehabilitation potential, and occupational therapy needs.
• Develop, implement, and modify individualized treatment plans in collaboration with the interdisciplinary care team.
• Establish therapy goals and communicate treatment recommendations, visit frequency, and progress updates to physicians and other healthcare providers.
• Provide therapeutic interventions, including ADL training, transfer/mobility training, neuromuscular re-education, therapeutic exercise, and balance training.
• Educate patients, families, and caregivers on treatment plans and home exercise programs.
• Maintain timely, accurate clinical documentation within the EMR system.
