Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides service within the scope of practice defined by state laws, in accordance with the plan of care and evidence-based techniques.
• Evaluates patient functional status and CMS-documented needs; consults with physician on therapy plan.
• Observes, records, and reports patient responses and changes to condition to nurse supervisor and physician.
• Conducts patient assessments using diagnostic and prognostic functional ability tests.
• Function in diverse home settings regardless of age, race, color, creed, sex, disability, or financial condition.
