Job summary

VNA OF HANOVER & SPRING GROVE seeks an experienced Home Health Occupational Therapist to provide patient-centered care in patients' homes. You will assess functional needs, develop individualized plans, and guide patients toward independence with ADLs and IADLs.

You will collaborate with a multidisciplinary team, educate caregivers, and document progress while adhering to agency policies and regulatory standards. Travel across Adams County/York region is required.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current OT license in the state of practice.
• CPR certification and valid driver's license.

Responsibilities

• Perform comprehensive occupational therapy evaluations in patients' homes.
• Develop individualized treatment plans based on physician orders and patient goals.
• Provide skilled interventions to improve ADLs and IADLs.
• Assess home safety and recommend adaptive equipment and environmental modifications.
• Train patients and caregivers in energy conservation, fall prevention, and compensatory strategies.
• Collaborate with nurses, physical therapists, speech therapists, social workers, physicians, and caregivers to coordinate patient care.
• Monitor patient progress and adjust treatment plans as appropriate.
• Complete timely and accurate electronic documentation.
• Participate in interdisciplinary team meetings and case conferences.
• Ensure compliance with all agency policies, payer requirements, and regulatory standards.
