Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance or access to dependable transport.
• Ability to work in diverse home environments.

Responsibilities

• Assess and evaluate patient care needs related to functional status and ADLs.
• Evaluate CMS documentation needs and plan of care with team.
• Consult with physician to develop therapy plan of care.
• Observe and report patient responses and condition changes to nurse and physician.
• Conduct functional ability assessments using appropriate tests.
