You will join a care team to develop therapy plans, document assessments for CMS, and monitor patient progress, ensuring quality care and effective communication with physicians and family members.

Qualificações

• Must hold a current Occupational Therapy license in the state of practice.
• Requires a valid driver's license or access to reliable transportation.

Responsabilidades

• Provides service within the scope of practice as defined by state laws and plan of care.
• Evaluates functional status and CMS documentation; collaborates with physician.
• Observes and reports patient changes to nurse supervisor and physician.
• Conducts patient assessments using functional ability tests.
• Able to work in diverse home situations regardless of client background.
