Job description

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a full-time home health visit position. This office services adult and geriatric clients across Carroll County and Howard County, Maryland.

Must be available to see patients full-time, Monday to Friday, between 8:30 am and 5:00 pm.

As a home care Occupational Therapist, you will be a vital part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care at home.

Responsibilities:

• Make home visits to clients within designated territories.
• Perform evaluations of patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
• Assess cognitive, psychological, and motor performance; develop individualized intervention plans.
• Evaluate home environments for hazards or barriers to independence.
• Identify equipment needs and environmental modifications to improve outcomes.
• Continuously assess and update care plans, collaborating with nursing staff as appropriate.
• Educate patients and families on rehabilitative activities to promote health, safety, and independence.
• Document observations, interventions, and evaluations accurately using electronic tablets.

Qualifications:

• Current Maryland Occupational Therapist license.
• Minimum one year of recent OT experience; home care experience preferred but not required.
• Graduate of an accredited occupational therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficient computer skills; EMR experience preferred.
