Graham Healthcare Group is seeking a Home Health Occupational Therapist for a role servicing Fremont, OH to Sandusky, OH and surrounding areas. The position emphasizes delivering patient care plans within a team-based environment and requires travel to patient locations about 20% of the day.

The OT candidate should hold a BS or MS in Occupational Therapy, be licensed in the state, and bring at least one year of healthcare experience.

• Licensed Occupational Therapist issued by the state of practice.
• Minimum of one year healthcare experience.
• Valid driver’s license and ability to spend ~20% of the day driving to/from patient locations.
• Commitment to meeting critical charting deadlines.
• Ability to self-manage time and schedule.
• Experience with tablets, mobile phones and EMR software.

We are looking for compassionate occupational therapists with:

• Must possess a Bachelor of Science orMaster'sdegree in Occupational Therapy from a school with an Occupational Therapy curriculum accredited jointly by the Council of Medical Education of the American Medical Association in collaboration with the American Occupational Therapy Association
• Licensed Occupational Therapist issued by the state in which you will be working.
• Minimum of one year healthcare experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
• A commitment to consistently meet critical deadlines for charting
• The skills needed to self-manage your time and schedule
• Demonstrated experience with tablets, mobile phones and EMR software
