Job summary

Graham Healthcare Group is seeking an Occupational Therapist for a position in Plainwell, Michigan. The OT will partner with Mary Free Bed at Home to manage and deliver patient care plans in a team-based home health setting.

Qualifications

• Must have a degree in Occupational Therapy and a valid state license.
• Minimum of one year healthcare experience.
• Ability to drive to patient locations ~20% of the day.
• Strong time management and self-management skills.
• Experience with tablets, mobile devices and EMR systems.

Responsibilities

• Deliver and manage patient care plans as part of a home health team.
• Collaborate with Mary Free Bed at Home and Mary Free Bed Rehabilitation Hospital.
• Support patients in Plainwell and surrounding areas.
