AHN Healthcare@Home in Erie, PA seeks an Occupational Therapist (OT) to provide skilled home health care to patients in their homes. The role offers a $10,000 first-year success bonus and ongoing rewards as you grow with the company.

Deliver patient-centered care, conduct ADL-focused evaluations, and develop individualized treatment plans. You will educate patients and caregivers on adaptive techniques, safety, and energy conservation, while maintaining compliant EMR documentation and traveling

Qualifications

• Active OT license in the state you work.
• Minimum 1 year of OT experience delivering home health care.
• Valid driver’s license and ability to drive ~20% of workday.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team.
• Perform comprehensive patient evaluations focused on ADLs, upper extremity function, and home safety.
• Develop individualized treatment plans to promote independence and quality of life.
• Educate and empower patients and caregivers on adaptive techniques and home safety strategies.
• Document thoroughly and compliantly in the EMR.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life
• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

OT Qualifications

• Active Occupational Therapist, OT license in the state you work
• One year of experience as an Occupational Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
