Job summary

Residential Home Health and Hospice is hiring an Occupational Therapist (OT) for a home health position providing skilled care to patients in their homes in the Carlinville, Litchfield, Girard, Benld, Gillespie and surrounding areas.

This position offers a total bonus potential of up to $15,000, including a $10,000 first-year success bonus and a $5,000 sign-on bonus paid within the first 18 months, plus opportunities for career growth and a supportive, collaborative team environment.

Qualifications

• Active Occupational Therapist license in the state you work
• At least one year of OT experience delivering patient care
• Current driver’s license and ability to drive ~20% of the day

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team
• Perform comprehensive evaluations focusing on ADLs, upper extremity function, and home safety
• Educate patients and caregivers on adaptive techniques and safety strategies
• Ensure compliant EMR documentation and independent, yet supported, practice
• Travel to patient homes within the assigned territory

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

• Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life

• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines

• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone

• Travel to patient homes within assigned territory

OT Qualifications

• Active Occupational Therapist, OT license in the state you work

• One year of experience as an Occupational Therapist and the clinical expertise to deliver quality patient care

• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
