Job summary

PurposeCare is seeking qualified Home Health Occupational Therapists to provide high-quality care for our community's most vulnerable clients in their homes. You will assess, plan, and implement OT services, document findings, and coordinate with families and care teams.

Qualifications

• Must be a licensed Occupational Therapist by the state.
• At least one year of clinical experience.
• Willingness to travel to patient residences.
• Valid driver's license and proof of car insurance.

Responsibilities

• Administers and writes evaluations of assigned patients in accordance with agency policy and procedures.
• Maintains timely and accurate patient records and reports.
• Coordinates with caregivers and other health professionals to ensure continuity of care.

Qualifications and Requirements:

• Valid driver's license and proof of car insurance
• Administers and writes evaluations of assigned patients in accordance with agency policy and procedures
• Must be a licensed Occupational Therapist by the state
• Maintains timely and accurate patient records and reports
• Must have at least one-year clinical experience
• Checks in on Certified Occupational Therapy Assistants after every 4-6 patient visits
• Must be able to travel to prospective patient's place of residence
