Graham Healthcare Group is hiring an Occupational Therapist for a Home Health position in Bellefonte, PA. You will deliver skilled, patient-centered care at patients’ homes, assess ADLs, educate on safety and adaptive techniques, and collaborate with a caring interdisciplinary team.

We offer a generous bonus structure ($15,000 total) including a $10,000 first-year success bonus and a $5,000 sign-on, with continued rewards as you grow with us. Travel is part of the role.

Competenze

• Active OT license to practice in the state you work.
• Minimum of 1 year OT experience delivering patient care.
• Valid driver’s license with ability to drive to patient locations (~20%).

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

• Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life

• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines

• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone

• Travel to patient homes within assigned territory

OT Qualifications

• Active Occupational Therapist, OT license in the state you work

• One year of experience as an Occupational Therapist and the clinical expertise to deliver quality patient care

• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
