AdvisaCare in Michigan is seeking compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to deliver high-quality care in patients' homes. You’ll perform Start of Care visits, routine visits, and assessments with clinical excellence while building strong, trusting relationships with patients and families.

This role emphasizes independent work, collaboration with a supportive team, and plans that empower patients.

Formación

• Michigan RN or LPN license is required.
• Experience performing Start of Care (SOC) visits and assessments.
• Excellent communication to educate patients and families.
• Home health or hospice experience preferred but not required.
• Ability to work independently and collaborate with physicians and therapists.
• Reliable transportation and eligibility to work in the United States.

Responsabilidades

• Complete Start of Care (SOC) visits with confidence and clinical excellence.
• Perform routine patient visits.
• Conduct assessments reflecting patient needs.
• Create individualized care plans for outcomes.
• Educate patients and families to empower them.
• Collaborate with a supportive team valuing your judgment and expertise.
• Document in the EMR with a smooth process.
• Be the RN who makes patients feel better.
