Job summary

AdvisaCare is seeking compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide high-quality nursing care in patients' homes across Michigan. Join a team that values independence, patient relationships, and clinical excellence.

You’ll perform SOC visits, routine patient visits, and assessments; create individualized care plans; educate patients and families; and collaborate with physicians and therapists for optimal outcomes.

Qualifications

• Active RN or LPN license in Michigan.
• Strong assessment skills and clinical judgment, including OASIS experience (RN) and follow-up visits.
• Strong communication skills, including educating patients and families, explaining care plans.
• Home health and hospice experience preferred but not required.
• Experience collaborating with physicians and therapists.
• Reliable transportation and eligibility to work in the United States.
• Experience with Kinnser is a plus.

Responsibilities

• Completing Start of Care (SOC) visits with confidence and clinical excellence.
• Completing routine patient visits.
• Performing assessments reflecting patient needs.
• Creating individualized care plans to drive outcomes.
• Educating patients and families to empower them.
• Collaborating with a supportive team.
• Documenting in the EMR with ease.
• Be the RN who makes patients say, “Wow… I feel better already.”
