Job summary

AdvisaCare in Michigan is seeking a compassionate Home Health or Hospice RN to join our team. You will deliver one-on-one care, assess patients, and collaborate with a supportive, team-focused group across communities including Traverse City, Kalkaska, and surrounding areas.

Qualifications

• Current Michigan License in good standing as an RN.
• Minimum 1 year in home health or hospice care.
• Empathetic approach to patients and caregivers.
• Ability to participate in field visits daily and attend meetings as needed.
• Adapt to patient status and needs.

Responsibilities

• Conduct initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs.
• Implement and revise the individualized plan of care.
• Ensure compliance with AdvisaCare policies and Medicare requirements.
• Educate patients and families on disease processes, self-care, and end-of-life care.
• Supervise paraprofessionals delivering services.
• Prepare clinical notes and participate in IDG conferences.
