Job summary

Homecare with Heart is seeking a compassionate Home Care Nurse to provide high-quality in-home care in the Leetonia, OH area. Starting pay for LPNs is $26 per hour and $30 per hour for RNs, with a $1,500 sign-on bonus and a schedule including every other Saturday 8a–5p and every other Sunday 11a–4p.

Responsibilities include medication administration, documentation, and collaboration with the clinical team to ensure an optimal Plan of Care.

Qualifications

• Must hold a valid LPN or RN license in the state of Ohio.
• Must have a valid driver's license and access to a vehicle.
• Ability to read Plan of Care, charts and agency policies.

Responsibilities

• Administer medication in accordance with standards of practice.
• Document care provided and maintain accurate progress notes.
• Monitor client conditions and report changes to supervisor.
• Collaborate with the clinical team to develop and improve the Plan of Care.
• Promote self-care and independence for clients at home.
• Maintain license, CEUs, and required employment documents.
