Job summary

Homecare with Heart in Ohio is seeking compassionate Home Care Nurses (LPN/RN) to oversee in-home care for clients. Start rates: LPN $26/hr, RN $30/hr, with a $1,500 sign-on bonus. The role involves administering medications, documenting care, and coordinating with the clinical team to implement the Plan of Care.

Travel is required within the service area. Qualified applicants will hold a valid LPN/RN license, a valid driver's license, and have Oasis experience.

Qualifications

• Valid State LPN/RN License required.
• Valid driver's license and access to a vehicle due to travel.
• Oasis experience is required.

Responsibilities

• Administer medication according to standards of practice.
• Document care and progress notes.
• Monitor client condition and report changes to supervisor.
• Provide skilled nursing visits in patients' homes.
• Maintain license, CEUs, and employment requirements.
• Collaborate with the clinical team to update the Plan of Care.
