Job summary

Healthcare Plus is seeking a Registered Nurse to provide in-home health care services, perform assessments, develop and revise care plans, administer treatments, and coordinate with physicians. This role requires strong communication and organizational skills, CPR certification, and a valid driver’s license with a reliable vehicle.

The RN will collaborate with families and care teams to support patient health goals, ensure safety, and manage discharge planning as needed during home visits.

Qualifications

• RN license required in the state where you practice.
• CPR certification required.
• Must have a valid driver’s license and reliable insured vehicle.

Responsibilities

• Complete initial patient and family assessment for home care.
• Re-evaluate patient nursing needs regularly.
• Initiate and revise plan of care as Patient status changes.
• Administer medications and treatments as prescribed.
• Counsel patient and family on nursing care needs.
• Coordinate patient care and communicate with physicians.
