Job summary

Lifecare Home Health is seeking a compassionate Registered Nurse to administer care in patients' homes, coordinate with the interdisciplinary team, and support families and referring agencies. You will perform ongoing assessments, adjust the care plan, and ensure quality and continuity of home health services.

The role requires an active RN license, CPR, a valid driver’s license, and excellent communication and decision-making skills.

Qualifications

• RN with valid license in any state
• CPR certification current
• English literacy required

Responsibilities

• Promote and embody company mission, vision, values
• Complete initial and ongoing patient assessments (including OASIS)
• Re-evaluate nursing needs and outcomes of care
• Develop and revise Plan of Care
• Communicate with patients, families, and healthcare team
• Apply infection control and universal precautions
• Coordinate changes with physicians and staff
• Monitor cases for third-party payer compliance
• Complete timely documentation
• Supervise and mentor other nursing personnel
• Provide on-call services
• Engage in continuing education and maintain competency
• Meet licensing board requirements
