Job summary

CarePartners is seeking a Registered Nurse to provide coordinated skilled nursing care in the home for patients of all ages. You will collaborate with the interdisciplinary team to meet patient goals and ensure high-quality care under the Clinical Manager.

Responsibilities include completing OASIS and visit documentation, communicating significant findings to the physician, and supervising home health aides and LVNs/LVNs.

Qualifications

• Education: Associate's Degree or Bachelor of Science in Nursing (BSN)
• License: Must have and maintain current RN license with NC Board of Nursing (NCBON); eNLC may apply; license to be obtained within 30 days
• Certification: BLS Healthcare Provider
• Experience: One year home health experience

Responsibilities

• Assesses home care patients identifying needs and maintains documentation
• Completes OASIS, assessment and visit paperwork per agency policy
• Communicates findings to Clinical Manager and physician; documents communications and interventions
• Supervises and directs home health aides and LPNs/LVNs to ensure quality and continuity of services
• Participates in on-call rotation and emergency calls per policy
