Job summary

CarePartners is seeking a Registered Nurse to provide coordinated skilled nursing care in patients' homes throughout western North Carolina. You will collaborate with an interdisciplinary team to set and achieve patient goals while maintaining high-quality care based on physician orders and clinical guidelines.

You will perform home health visits, complete OASIS documentation, and communicate significant findings to the Clinical Manager and physician.

Qualifications

• Associate's degree or BSN in Nursing.
• Current RN licensure with the North Carolina Board of Nursing; eNLC may apply; license must be obtained within 30 days of employment start date.
• BLS Healthcare Provider certification.
• One year home health experience.

Responsibilities

• Assess home care patients and identify physical, psychosocial and environmental needs.
• Complete OASIS, assessment and visit paperwork per agency policy; document coordination with physicians and team.
• Communicate significant findings to Clinical Manager and physician; document all findings and interventions.
• Supervise home health aides and LVNs/LPNs to ensure quality and continuity of services.
• Participate in on-call rotation and emergency calls per agency policy.
