Talin Home Healthcare LLC is seeking multiple compassionate Home Health Nurses to serve patients across the Rappahannock County area. You will provide skilled nursing care in patients’ homes throughout Northern Virginia, including wound care, IV therapy, and ostomy care, while building trusted relationships with families.

Ideal candidates will have strong clinical assessment skills, excellent communication, and the ability to work independently with a flexible schedule.

Responsabilidades

• Provide skilled nursing care to patients in their homes across the NOVA region.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Monitor vital signs and document patient progress.
• Educate patients and families on disease management, medications, and home care techniques.
• Collaborate with physicians and care team to ensure coordinated care.
• Maintain accurate clinical documentation in line with regulatory standards.

Talin Home Healthcare LLC is looking for multiple compassionate and dedicated Home Health Nurses to serve patients across Rappahannock County area. Responsibilities:

• Provide skilled nursing care to patients in their homes across the Northern Virginia area
• Conduct comprehensive patient assessments and develop individualized care plans
• Must have experience with administering medications, wound care wet and dry, wound vac, IV therapy, ostomy care and other clinical treatments as ordered.
• Monitor and document patient progress, vital signs, and changes in condition
• Educate patients and family members on disease management, medications, and home care techniques
• Collaborate with physicians, therapists, and care team members to ensure coordinated patient care
• Maintain accurate and timely clinical documentation in compliance with regulatory standards

Requirements:

• Active RN license in the state of Virginia
• Previous home health, hospital med-surg, or community nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license for travel throughout the NOVA region
• Ability to work independently and manage a flexible patient caseload
• Compassionate, patient-centered approach to care
• Must commit to working a minimum of 2 days a week 1 day of the days must be a Monday, and the other can be Wednesday or Friday.
