Job summary

Beth Israel Lahey Health at Home is seeking an experienced Home Health Registered Nurse to plan, direct, and coordinate patient care in patients' homes across a tight territory. The role emphasizes community health education and adherence to home care regulations and payer guidelines.

The ideal candidate holds a Massachusetts RN license with BLS, at least 1 year of direct care experience (preferably home health), and familiarity with OASIS and EPIC.

Qualifications

• Active Massachusetts RN license and current BLS certification.
• Associate’s Degree in Nursing or BSN; BSN preferred.
• At least 1 year of direct care nursing experience, home health preferred.
• OASIS and EPIC experience are preferred.

Responsibilities

• Complete and document an initial patient/family assessment to plan home care.
• Initiate and revise the care plan as patient needs change.
• Administer medications and treatments per physician orders.
• Instruct, supervise, and evaluate the home health aide care every two weeks.
• Identify discharge planning needs and ensure HIPAA confidentiality.
