Job summary

Beth Israel Lahey Health, Inc. is seeking aRegistered Nurse to plan, organize and direct home care services, utilizing community resources to meet patient and family needs in homes and communities in accordance with home care regulations.

The role emphasizes initial assessments, plan of care development, medication administration, and supervising home health aides. OASIS and EPIC proficiency are preferred. A competitive salary and comprehensive benefits are offered.

Qualifications

• Massachusetts RN license active
• Active BLS certification
• Associate’s Degree or BSN (preferred)
• At least 1 year of direct care nursing experience (public health/home care preferred)
• OASIS and/or EPIC experience preferred

Responsibilities

• Complete and document initial patient and family assessment to determine home care needs.
• Initiate and revise the plan of care as patient status changes.
• Administer medications and treatments as prescribed.
• Instruct, supervise, and evaluate home health aide care every two weeks.
• Identify discharge planning needs as part of care planning.
• Maintain HIPAA confidentiality.
• Complete OASIS documentation and EPIC records promptly.

What You’ll Do:

The registered nurse plans, organizes and directs home care services and has experience in nursing with emphasis on community health education. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of patients and families within their homes and communities in accordance with home care regulations and payer guidelines.

• Complete and document an initial assessment of patient and family to determine home care needs;
• Initiates the plan of care and makes necessary revisions as patient status and needs change;
• Administers medications and treatments as prescribed by the physician;
• Instruct, supervise, and evaluate the home health aide care provided every two (2) weeks;
• Identify discharge planning needs as part of the care plan development;
• Practice confidentiality principles set by the agency and Federal HIPAA guidelines;
• Demonstrate proficiency in OASIS completion;
• Complete documentation through EPIC in accordance with agency timeliness policies.

What You’ll Need:

• An active valid Massachusetts RN license;
• An active American Health Association BLS certification;
• Associate’s Degree or BSN (preferred) from an accredited school of nursing;
• At least 1 year of direct care nursing experience, preferably the area of public health or home care nursing;
• OASIS and/or EPIC experience is preferred.

• Pay Range: $63,200.00 USD - $91,920.00 USD
• The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
