Job summary

Optum Care at Home in Thurston County, WA is seeking a nurse practitioner or physician assistant to deliver in-home care. You will perform histories, exams, and treatments, coordinate with an interdisciplinary team, and travel daily to patient homes to support high-risk individuals.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Current NCCPA certification and state licensure.
• Active, unrestricted licensure in the practicing state without disciplinary actions.
• Active and unrestricted driver's license with reliable transportation.
• DEA licensure or ability to obtain post-hire per state regulations.
• Ability to gain a collaborative practice agreement, if applicable.
• Willingness to travel daily to patient homes.

Responsibilities

• Past medical history review, medication reconciliation, vital signs, and physical exam.
• Evidence-based screenings and point-of-care testing as appropriate.
• Manage members through acute medical situations or chronic exacerbations.
• Ensure accurate and complete documentation including ICD-10.
• Communicate and collaborate with the interdisciplinary care team.
• Conduct advanced illness and end-of-life conversations.
• Provide patients and caregivers with counseling and education.
