Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Attleboro, MA.

Qualifications

• Masters degree in nursing or equivalent with NP licensure.
• Active State NP license and current national board certification.
• Current BLS certification.
• At least 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document accurately.
• Conduct medical reconciliation for safety and compliance.
• Provide basic wellness education on prevention and health maintenance.
