Job summary

Your Health Organization seeks a Nurse Practitioner to provide in-home primary care across the Athens area. As a Provider, you collaborate with a multidisciplinary team to evaluate high-risk patients, develop care plans, and manage complex cases, ensuring high-quality, patient-centered outcomes.

This full-time, salary-based role offers a monthly bonus and shifts including 12-hour 7-on/7-off or 8-hour Mon-Fri. Travel between offices, facilities, and patient homes is required.

Qualifications

• Licensed NP/PA/Physician in the employment state in good standing.
• Experience in patient care, preferably home health, clinics, or telehealth.
• Excellent communication and collaboration within an interdisciplinary team.
• Ability to evaluate high-risk patients and develop care plans.
• Commitment to compassionate, patient-centered care and excellence.
• Strong organizational and time management skills.
• Ability to read and communicate effectively; written and verbal skills strong.
• Basic computer knowledge; leadership and teamwork abilities.
• Willingness to travel between offices and patient homes; valid driver's license.

Responsibilities

• Evaluate high-risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with interdisciplinary team to ensure continuity of care and outcomes.
• Communicate with case managers, coordinators, and other professionals to coordinate patient care.
• Participate in team meetings to discuss patient progress and care coordination.
• Document care management activities to support plans and regulatory compliance.
• Provide education and resources to patients on conditions, treatment plans, and self-management.
• Offer support to patients and families to improve understanding of options and participation.
• Adhere to HIPAA, Medicare guidelines, and other healthcare regulations.
