Qualifications

• Current unrestricted license to practice as a registered professional nurse.
• Current unrestricted license to practice as a Nurse Practitioner.
• Current and continued CPR certification.
• Valid driver’s license for work travel.

Responsibilities

• Initiates palliative care consultations or hospice treatment plans.
• Consults with medical colleagues as needed and documents timely.
• Provides patient with health education and counseling.
• Participates in the interdisciplinary team as needed.
