Job summary

WellBe Senior Medical in Charleston, WV is seeking a Nurse Practitioner for in-home adult medicine. The role is field-based with full benefits and relocation support, offering autonomy to deliver primary care at patients' homes.

You will manage chronic conditions, collaborate with an interdisciplinary team, and aim to prevent ER visits by providing comprehensive, home-based care and care coordination.

Responsibilities

• Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the home.
• Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients.
• Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions.
• Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.

Education

Master’s Degree (NP) with focus on Acute Care, Adult, Geriatric, and/or Family
