The role includes managing chronic disease with longitudinal care plans, collaborating with an interdisciplinary team, and reducing ER visits by treating acute needs at home; requires NP masters, active license and state certification, and valid driver's license.

Qualifications

• Master’s Degree (NP) with focus on Adult, Acute Care, Geriatric, and/or Family.
• Active, unencumbered NP license and Board Certification (ANCC or AANP).
• Valid driver’s license and vehicle for local field-based travel.

Responsabilités

• Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home.
• Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients.
• Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions.
• Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.
