Job summary

WellBe Senior Medical in Atlanta, GA is seeking a Nurse Practitioner (APP) to deliver field-based primary care visits in patients' homes. You’ll manage chronic conditions, create longitudinal care plans, and work with an interdisciplinary team to ensure seamless transitions of care.

You will operate with autonomy, address social determinants of health, and contribute to a high-value, home-based care model that emphasizes quality and patient satisfaction.

Qualifications

• Master’s Degree in Nursing (NP) required.
• GA State NP license and national board certification (ANCC or AANP) required.

Responsibilities

• Deliver Field-Based Care: conduct comprehensive primary care visits and exams in patients' homes.
• Manage Chronic Disease: create longitudinal care plans for high-risk patients.
• Collaborate Across the Continuum: partner with an interdisciplinary team for seamless care transitions.
• Prevent ER Visits: treat acute needs in the home to keep patients safe and at home.
