Job summary

AdvisaCare Home Health and Hospice seeks a full-time MSW to join our compassionate team in Michigan. The MSW will perform psychosocial assessments, develop person-centered care plans, and provide counseling and bereavement support under physician supervision.

Qualified candidates have an MSW from a CSWE-accredited program, at least one year in home health, hospice experience preferred, and a valid driver's license with reliable transportation.

Qualifications

• MSW degree from CSWE-accredited school.
• One year of Home Health Care experience preferred.
• Hospice experience preferred.
• Valid Driver's License and reliable transportation.

Responsibilities

• Performs psychosocial, emotional, spiritual and bereavement assessments and helps in goal-directed care plans.
• Provides care under the direction of a physician.
• Participates in IDT conferences, sharing psychosocial information affecting patient and family.
• Documents all patient/family visits, calls and referrals in the chart.
• Conducts reassessments of patient/family needs and counseling as required.
• Provides crisis intervention and individual or family counseling.
• Acts as a care advocate for patient/family within the team.
• Maintains contact with community agencies to promote referrals.
• Offers ongoing counseling on death and dying as needed.
• Participates in Bereavement Team activities as assigned.
• Adheres to all Hospice policies.
