Job summary

Guardian Angel Home Care is seeking a PRN Medical Social Worker (MSW) to join our home health team. You will perform psychosocial assessments, counseling, and connect patients with community resources while traveling to patients' homes within the service area.

The role offers flexible, per-visit scheduling with competitive compensation and opportunities for additional assignments based on census and coverage needs. MSW license required, driver’s license essential.

Qualifications

• Master’s Degree in Social Work (MSW) — REQUIRED
• Full, active, unrestricted social work license — REQUIRED
• Limited licenses will not be accepted
• Experience in home health, hospice, hospital, rehabilitation, or community-based preferred
• Strong knowledge of community resources
• Excellent communication and documentation skills
• Ability to work independently in patients’ homes
• Reliable, compassionate, and professional

Responsibilities

• Complete psychosocial assessments
• Evaluate patient and family needs
• Provide counseling and emotional support
• Connect patients with community resources and support programs
• Assist with financial, transportation, housing, and long-term care resources
• Support patients and families with adjustment to illness or disability
• Participate in interdisciplinary care planning
• Coordinate with nurses, therapists, physicians, and other clinical staff
• Document visits and interventions accurately and timely
• Travel to patients’ homes throughout the assigned service area
