Hill Country preferred Senior Care Home Health is seeking a skilled Home Health LVN to join our team in Junction, TX. The LVN will provide nursing care to patients at home under supervision of a RN or physician, delivering compassionate, patient‑centered care.

The ideal candidate has Texas LVN license, CPR certification, at least one year in home health, strong communication, and reliable transportation; a competitive benefits package accompanies this full‑time role.

Qualifications

• Texas LVN license required.
• CPR certification required.
• Minimum one year experience in home health care.
• Good communication and interpersonal skills.
• Valid driver's license and reliable transportation.
• Ability to pass background check and drug screen.

Responsibilities

• Assess patients' health status and develop care plans
• Administer medications and treatments as prescribed by a physician
• Monitor patients' vital signs and report any changes to the RN or physician
• Provide wound care and other nursing interventions as needed
• Document patient care and progress in electronic medical records
• Communicate with patients, families, and other healthcare providers as needed
• Participate in interdisciplinary team meetings and case conferences
• Adhere to all state and federal regulations and agency policies and procedures
