Hill Country preferred Senior Care Home Health is seeking a licensed vocational nurse (LVN) for full-time home health assignments in Mason, Texas. The role involves delivering skilled nursing care to patients in their homes under RN/physician supervision, with an emphasis on compassion and clear communication.

Qualifications include a Texas LVN license and CPR, at least 1 year in home health, and strong teamwork and critical thinking abilities.

Qualifications

• Current LVN license in Texas and CPR certification are required.
• Minimum of one year of home health care experience.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Strong critical thinking and problem-solving skills.
• Valid driver’s license and reliable transportation.
• Ability to pass a background check and drug screening.
• Wound care 2-3x per week.

Responsibilities

• Assess patients’ health status and develop care plans.
• Administer medications and treatments as prescribed.
• Monitor vital signs and report changes to RN or physician.
• Provide wound care and other nursing interventions.
• Document patient care in electronic medical records.
• Communicate with patients, families, and healthcare providers.
• Participate in interdisciplinary team meetings and case conferences.
• Adhere to state/federal regulations and agency policies.
