Job summary

Hill Country preferred Senior Care Home Health is seeking a Home Health LVN in Junction, TX to provide nursing care to patients in their homes under the direction of an RN or physician.

The ideal candidate will hold a Texas LVN license with CPR certification and have at least 1 year of home health experience. Strong communication, independence, and teamwork skills are essential to deliver quality home health services and to participate in interdisciplinary meetings and compliant care planning.

Qualifications

• Current TX LVN license and CPR certification.
• Minimum 1 year of experience in home health care.
• Excellent communication and interpersonal skills.
• Ability to pass a background check and drug screening.

Responsibilities

• Assess patients' health status and develop care plans
• Administer medications and treatments as prescribed by a physician
• Monitor patients' vital signs and report any changes to the RN or physician
• Provide wound care and other nursing interventions as needed
• Document patient care and progress in electronic medical records
• Communicate with patients, families, and other healthcare providers as needed
• Participate in interdisciplinary team meetings and case conferences
• Adhere to all state and federal regulations and agency policies and procedures
