Job summary

DFW Home Health, part of LHC Group and within the Optum family, is hiring a full-time Home Health LVN to join our caring team serving Fort Worth and the Crowley area. This role emphasizes quality patient care in patients' homes, with support for care coordination and regulatory adherence.

The LVN will perform skilled nursing duties, document visits, and coordinate with the health care team to ensure optimal outcomes. A valid LVN license and reliable transportation are required.

Qualifications

• Current LVN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within scope of practice and plan of care, coordinated with the care team.
• Documents patient visits thoroughly in the electronic medical record per payer requirements.
• Participates in care coordination, reports changes in patient condition, and communicates with patient, family, and clinicians.
• Function effectively in homes regardless of age, race, creed, or disability.
