DFW Home Health, part of LHC Group, is hiring for a full-time LVN in Fort Worth, TX to provide high-quality home health care. You will follow the plan of care and coordinate with the health care team to support patient needs.

Requirements include current Texas LPN licensure and a valid driver’s license. CPR certification is preferred; strong communication and flexibility are valued, with a competitive benefits package.

Qualifications

• Current and unrestricted LPN licensure in state of practice (Texas).
• Current driver’s license and vehicle insurance, access to a dependable vehicle or public transportation.
• CPR certification or ability to complete within 90 days of hire.
• Ability to manage multiple tasks simultaneously.
• Able to work independently.
• Good communication, writing, and organizational skills.
• Flexible hours as required to meet client needs.

Responsibilities

• Provides high quality clinical services within scope of practice and plan of care, in coordination with the health care team.
• Documents patient visits thoroughly in the electronic medical record per regulatory requirements.
• Participates in care coordination, reporting patient needs and changes in condition to the team.
• Functions in diverse home settings regardless of age, race, creed, or disability.
