Job summary

DFW Home Health is hiring a full-time Licensed Vocational Nurse (LVN) to provide skilled nursing care in the patient’s home, covering the Crowley area from Fort Worth. The ideal candidate holds a current LVN license, a valid driver’s license, and durable communication and organizational skills.

Qualifications

• Current and unrestricted LVN licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.
• Ability to manage multiple tasks simultaneously.
• Able to work independently.
• Good communication, writing, and organizational skills.
• Ability to work flexible hours as required to meet identified client needs.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws and plan of care, coordinating with the health care team.
• Documents patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Participates in the agency’s care coordination process and communicates patient needs and changes in condition.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition.
