Job summary
Kona Medical Consulting and The Home Health Team is actively looking for a Home Health Field Licensed Vocational Nurse (LVN) in the Dallas–Fort Worth Metroplex. The LVN will provide skilled nursing care in patients' homes under supervision of an RN Case Manager, perform wound care, med administration, vital signs, and care plan updates, and travel within the service area.

Qualifications
- Current Texas LVN license.
- 1+ year of nursing experience (Home Health preferred).
- Strong assessment, communication, and clinical skills.
- Reliable transportation, valid driver’s license, and auto insurance.
- Familiarity with EMR/mobile charting.
- Compassionate, dependable, and patient-focused.
Responsibilities
- Provide skilled nursing care under the supervision of an RN Case Manager.
- Perform wound care, medication administration, vitals, and disease process teaching.
- Assist with care plan implementation and updates.
- Conduct patient assessments and report changes in condition promptly.
- Collaborate with physicians, RNs, and other team members to ensure quality care.
- Maintain timely, accurate documentation in the EMR.
- Travel to patient homes within an assigned service area.
