Job summary

Optum, through Elite Home Health, part of LHC Group, is hiring a Per Diem LPN/LVN to treat patients in their homes two days per week in Mountain Home, AR. You will follow the plan of care, coordinate with the care team, and deliver skilled nursing services in the patient’s home.

Requirements include current and unrestricted LPN/LVN licensure, a valid driver’s license with reliable transportation, and CPR certification (preferred).

Qualifications

• Current and unrestricted LPN/LVN license in state of practice.
• Current driver's license with reliable vehicle or public transportation.
• CPR certification preferred or able to obtain within 90 days.

Responsibilities

• Provide skilled nursing services per plan of care in the patient’s home.
• Instruct family on home care and healthy environment for the patient.
• Monitor patient response and report changes to supervising staff.
