Job summary

CHRISTUS HomeCare, part of the LHC Group, is hiring a Licensed Practical Nurse for home health to provide quality patient care in patients' homes, following their plan of care and infection control procedures.

Qualifications include active LPN licensure in the state, a valid driver's license, and access to reliable transportation. Excellent communication and organizational skills are required for successful coordination of care.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provide high quality clinical services within the scope of practice and infection control procedures.
• Document patient visits thoroughly in the electronic medical record per regulatory requirements.
• Participate in care coordination, report patient needs and changes in condition, attend case conferences, and communicate with patients, families, physicians, and other clinicians.
