Job summary

St. Luke's University Health Network is seeking a Licensed Practical Nurse to join the Home Health team. You will work as part of an interdisciplinary group to provide high-quality nursing care in patients' homes or inpatient units, following physician orders and agency policies.

Qualifications

• Graduate of a School of Practical Nursing with a current license as a Practical Nurse.
• Minimum of two years experience as an LPN.
• Ability to collaborate with an interdisciplinary team and follow physician orders.

Responsibilities

• Collaborates with interdisciplinary team to assure appropriate patient care and care planning.
• Provides comprehensive nursing care and health teaching under direction of RN/Physician.
• Maintains patient records per agency guidelines and ensures documentation meets policies.
