Job summary

VitalCaring Group seeks a Licensed Practical Nurse (LPN/LVN) to provide direct, compassionate home health care to patients in their homes across the assigned territory. You will deliver skilled nursing services under RN supervision, reinforce education, and document care in the EMR to support optimal outcomes.

The role is field-based with a flexible schedule, allowing you to build meaningful patient relationships while maintaining clinical excellence and adherence to the Nurse Practice Act.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one year nursing experience preferred (Home Health, Med‑Surg, or Acute Care a plus).
• Strong clinical and observational skills.
• Clear communication and patient education abilities.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act
• Implement and support the plan of care developed by the RN
• Deliver skilled nursing services, including treatments, medication support, and wound care
• Monitor patient condition and report changes to the RN and care team
• Educate patients and caregivers on self‑care, treatments, and disease management
• Maintain infection control and proper use of equipment and supplies
• Document patient care accurately and timely within the EMR
• Communicate effectively with physicians, nurses, and interdisciplinary team members
• Participate in case conferences, team meetings, and ongoing training
• Support coordinated care by maintaining consistent communication across the care team
