Job summary

VitalCaring Group is seeking a Licensed Practical/Vocational Nurse (LPN/LVN) for Field-Based Home Health. You will provide direct patient care under RN supervision, support care plans, and help patients heal and maintain independence in their homes.

You will deliver skilled nursing services, monitor conditions, and educate patients and caregivers to improve outcomes while collaborating with an interdisciplinary team.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one (1) year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).
• Strong clinical and observational skills.
• Ability to follow care plans while reporting changes in condition.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and support the plan of care developed by the RN.
• Deliver skilled nursing services, including treatments, medication support, and wound care.
• Monitor patient condition and report changes to the RN and care team.
• Educate patients and caregivers on self-care, treatments, and disease management.
• Maintain infection control and proper use of equipment and supplies.
• Document patient care accurately and timely within the EMR.
• Communicate effectively with physicians, nurses, and interdisciplinary team members.
• Participate in case conferences, team meetings, and ongoing training.
• Support coordinated care by maintaining consistent communication across the care team.
